Day Program Support Specialist

  

The Day Program Support Specialist provides person-centered support to individuals with intellectual and developmental disabilities to facilitate meaningful engagement in community life. This role helps clients build relationships, develop natural supports, access community resources, and participate in recreational, educational, volunteer, and social activities. The Day Program Support Specialist works closely with participants, families, guardians, and community partners to support inclusion, skill-building, and independence.


Classification:

-Full-time/Part-time, Hourly, non-exempt

-Monday through Friday 

-$17/ hour


Ability:

- Must work well independently and within a team environment

-Must communicate professionally with interdisciplinary team members

-Must maintain driver standards per insurance and State requirements

-Must pass background checks in accordance with company policy and State regulations

-Ideal candidate is creative and engaged in community and willing to share ideas with others

Physical Requirements (with or without reasonable accommodation):

-Must be able to lift at least 50lbs.

-Must be able to stand, walk, climb, kneel, stoop, bend, crouch, reach, grasp, pinch and twist

-Must be able to drive company vehicle and operate wheelchair lift, including the loading and unloading of passengers

Certifications/Education:

-Must be at least 21 years of age

-Must possess valid Colorado Driver’s license

-Must obtain and submit a satisfactory Driving Record upon hire and annually/ as requested thereafter. 

-High School Diploma or GED

-May require successful completion of QMAP course within 10 days of hire (training provided by employer)

-Must maintain CPR/First Aid certification (training provided by employer)

-Must successfully complete Core In-service training and on-going training as required (training provided by employer)

-Must communicate effectively in English both verbally and in writing

-Must possess basic math skills including addition, subtraction, multiplication, division, and basic conversions.

-Must have a safe and reliable vehicle, and perform vehicle safety inspections in accordance with applicable rules and regulations.

-Be willing to transport individuals in personal vehicles

- Must maintain appropriate and valid vehicle insurance and registration

-Have a passion and dedication in caring and advocating for adults with disabilities


Job Duties:


Participant Support & Community Engagement

● Collaborate with participants to identify interests, goals, and preferred community activities.

● Plan, coordinate, and accompany participants to social, educational, recreational, and volunteer opportunities.

● Encourage participants to try new activities and expand social networks.

● Facilitate skill-building and promote natural supports within the community.

● Support participants in using public transportation or other safe travel methods.

● Ensure participant safety during outings and respond appropriately to emergencies.
 

Individual Planning & Documentation

● Contribute to person-centered service plans (ISPs, goal plans).

● Track participant progress and outcomes; maintain accurate documentation and reports.

● Communicate regularly with families, guardians, and case managers.

● Monitor barriers and propose creative solutions to support success.
 

Community & Partner Relations

● Build and maintain partnerships with community organizations and civic groups.

● Advocate for inclusion, accessibility, and accommodation for all participants.

● Represent the organization professionally within the community.
 

Compliance & Quality

● Follow all agency policies and procedures, as well as Colorado Medicaid waiver rules.

● Maintain confidentiality and uphold participant rights.

● Participate in training, meetings, and quality assurance activities.

This is a driving position that may require the use of your personal vehicle to transport individuals we serve, as well as use of a company vehicle. The State of Colorado requires that any vehicle used for this purpose pass a detailed multi-point safety inspection. Some of the items on the inspection include windshield, suspension, brakes, lights, seatbelts, and other safety features. If offered this position, C.A.R.E. Inc will reimburse the cost of the inspection, however any repairs required are at the expense of the applicant and must be resolved within 15 days of hire.
